Management of Vaginal Dryness in Female Patients

First-Line Treatment Approach

  • The American College of Obstetricians and Gynecologists recommends hormone-free lubricants and moisturizers as the first choice for treating vaginal dryness and soreness in female patients with schizoaffective disorder on lurasidone 1, 2, 3
  • Water-based lubricants are recommended for use during sexual activity in female patients with vaginal dryness 4
  • Silicone-based products may last longer than water-based or glycerin-based products and can be considered as an alternative for female patients with persistent vaginal dryness 3, 4
  • Regular use of vaginal moisturizers can provide ongoing relief from dryness and discomfort in female patients with schizoaffective disorder on lurasidone 4

Treatment Algorithm

  • The National Comprehensive Cancer Network recommends starting with hormone-free options, including daily vaginal moisturizers for maintenance and water-based lubricants for sexual activity, for female patients with vaginal dryness 1, 2, 4
  • If insufficient relief is achieved after 2-4 weeks, consideration of pelvic floor physical therapy and vaginal dilators may be necessary for female patients with persistent vaginal dryness 4, 5

Medication Considerations

  • The American College of Obstetricians and Gynecologists recommends low-dose vaginal estrogen therapy with caution in female patients with psychiatric disorders due to potential drug interactions, and consultation between psychiatrist and gynecologist is recommended 2, 4, 5

Follow-Up Recommendations

  • Regular assessment of response to treatment and overall sexual function is important for female patients with vaginal dryness, as it may be part of broader sexual dysfunction 3, 4, 5
