Hydralazine Dosing and Administration Guidelines

Introduction to Hydralazine Therapy

  • The American Heart Association recommends hydralazine 37.5 mg three times daily (in fixed-dose combination with isosorbide dinitrate), titrating to target dose of 75 mg three times daily for mortality benefit in heart failure patients 1

Oral Dosing for Hypertension and Heart Failure

  • For heart failure with reduced ejection fraction, the American College of Cardiology suggests initiating hydralazine at 37.5 mg three times daily, with a target dose of 75 mg three times daily for mortality benefit 1
  • The mean effective dose in trials for heart failure was approximately 175 mg daily (divided three times daily) 1
  • Three times daily dosing of hydralazine is supported by guidelines for heart failure applications 1

Intravenous Dosing for Acute Hypertension

  • For hypertensive emergencies, the American Heart Association recommends administering hydralazine 10-20 mg as a slow IV bolus, repeated every 4-6 hours as needed, though it is not a first-line agent 3, 4
  • The initial dose for IV administration is 10 mg slow IV infusion (maximum initial dose 20 mg) 3
  • Repeat dosing is every 4-6 hours as needed based on blood pressure response 3, 4

Pediatric Dosing

  • The initial dose for pediatric patients is 0.75 mg/kg/day 2
  • The maximum dose for pediatric patients is 7.5 mg/kg/day up to 200 mg/day 2
  • The frequency for pediatric patients is four times daily 2

Critical Monitoring Parameters

  • Blood pressure should be monitored at 10-30 minutes, 1 hour, and 2-4 hours after each dose, particularly during initiation and titration 1
  • Continuous monitoring is recommended for IV administration to prevent "overshoot" hypotension 3

Adverse Effect Monitoring

  • The risk of lupus-like syndrome increases significantly at cumulative daily doses >200 mg/day 1
  • Reflex tachycardia requires concomitant beta-blocker therapy 1
  • Fluid retention requires concomitant diuretic therapy 1

Common Pitfalls to Avoid

  • IV hydralazine should not be used as first-line for hypertensive emergencies unless other agents are contraindicated 4
  • Once-daily conventional hydralazine should not be used as it provides inadequate 24-hour control, especially in rapid acetylators 1
  • The dose should not exceed 300 mg daily without compelling indication due to increased lupus risk 1
  • Hydralazine should always be combined with beta-blocker and diuretic to counteract reflex tachycardia and fluid retention 1
