Ozempic Dosing Guidelines for Type 2 Diabetes and Weight Management
Standard Dosing Protocol for Type 2 Diabetes
- The American Diabetes Association recommends starting Ozempic at 0.25 mg subcutaneously once weekly for 4 weeks, then increasing to 0.5 mg weekly, with optional escalation to 1 mg weekly after at least 4 weeks if additional glycemic control is needed, and a maximum dose of 2 mg weekly for diabetes management 1, 2, 3
- The low starting dose minimizes gastrointestinal side effects, which is a common adverse effect of semaglutide 1, 3
- The standard maintenance dose for most patients is 0.5 mg once weekly 3
- Dose escalation to 1.0 mg weekly may be considered after at least 4 weeks at 0.5 mg if additional glycemic control is needed 1, 2, 3
- The maximum approved dose for diabetes management is 2.0 mg weekly 3
Administration Details
- Ozempic should be injected subcutaneously in the abdomen, thigh, or upper arm 1, 2, 3
- Administration should be once weekly at any time of day, with or without meals 1, 2, 3
- Injection should be on the same day each week 1
- Rotation of injection sites within one anatomical area is recommended to minimize absorption variability 3
Special Populations
- No dose adjustment is required for patients with renal impairment, including end-stage renal disease 1, 2, 3
- No dose adjustment is required for patients with hepatic impairment 2, 3
Concomitant Medication Adjustments
- When initiating semaglutide, the insulin dose should be reduced by 20% to prevent hypoglycemia 5
- Sulfonylurea doses should be considered for discontinuation or reduction due to hypoglycemia risk 3, 4
- Semaglutide should not be combined with other GLP-1 receptor agonists or DPP-4 inhibitors 3, 4, 5
Critical Contraindications
- Absolute contraindications include personal or family history of medullary thyroid cancer, multiple endocrine neoplasia syndrome type 2 (MEN2), pregnancy or lactation, and history of severe hypersensitivity reaction to semaglutide 1, 3, 4, 6
Monitoring Requirements
- During titration, patients should be assessed every 4 weeks for GI tolerance, weight, blood pressure, and signs of pancreatitis/gallbladder disease 5
- After reaching maintenance dose, patients should be monitored every 3 months for weight, blood pressure, cardiovascular risk factors, and medication adherence 4, 5
