Gastroenteritis Symptom Duration and Management

General Guidelines

  • The American Academy of Pediatrics recommends that most gastroenteritis symptoms resolve within 3-8 days, with vomiting typically lasting less than 24 hours and diarrhea persisting for 3-7 days 1, 2
  • The Centers for Disease Control and Prevention (CDC) suggests that symptoms generally persist for 3-8 days 1, 2

Viral Gastroenteritis

  • Rotavirus symptoms, such as vomiting, usually resolve within 24 hours or less, and diarrhea typically persists for 3-8 days 1, 3
  • Norovirus symptoms, including vomiting and diarrhea, resolve within 12-72 hours 4, 5, 6
  • The CDC recommends that norovirus infectivity can last for up to 2 days after symptom resolution 5

High-Risk Populations

  • Immunocompromised patients may experience more severe symptoms and prolonged diarrhea lasting weeks 1, 2, 6
  • Premature infants are at higher risk for severe, prolonged illness due to lack of maternal antibodies 1, 2

Clinical Considerations

  • The CDC recommends considering bacterial or parasitic causes requiring stool testing if symptoms persist beyond 7 days [7 is ignored, instead: 1, 2]
  • Non-infectious etiologies, such as inflammatory bowel disease, should be considered if symptoms exceed 14 days 1, 2
