Hyper-CVAD Treatment Protocol for Acute Lymphoblastic Leukemia

Introduction to Hyper-CVAD

  • The National Comprehensive Cancer Network recommends Hyper-CVAD, a regimen consisting of hyperfractionated cyclophosphamide, vincristine, doxorubicin, and dexamethasone (Cycle A) alternating with high-dose methotrexate and cytarabine (Cycle B), for a total of 8 cycles with mandatory CNS prophylaxis 1, 2

Treatment Structure

  • Vincristine is administered at 2 mg IV on days 4 and 11 of Cycle A 1
  • Dexamethasone is given at 40 mg PO or IV daily on days 1-4 and 11-14 of Cycle A 1, 2
  • Methotrexate is administered at 1 g/m² IV over 24 hours on day 1 of Cycle B 1
  • Cytarabine is given at 3 g/m² IV over 2 hours every 12 hours for 4 doses on days 2-3 of Cycle B 1

CNS Prophylaxis

  • The National Comprehensive Cancer Network recommends intrathecal therapy with methotrexate 12 mg and/or cytarabine 100 mg on day 2 of each cycle 1
  • Alternative triple intrathecal therapy (methotrexate, cytarabine, hydrocortisone) is also recommended 3, 5

Rituximab Addition for CD20-Positive Disease

  • For CD20-positive B-cell malignancies, the National Comprehensive Cancer Network recommends adding rituximab 375 mg/m² IV on days 1 and 11 of each Cycle A and day 2 of each Cycle B 1, 7
  • This modification improves 3-year complete remission duration to 67% and overall survival to 61% in Ph-negative B-ALL 7

Maintenance Therapy

  • The National Comprehensive Cancer Network recommends maintenance therapy with methotrexate 10 mg/m² PO weekly, 6-mercaptopurine 50 mg PO three times daily, and vincristine/prednisone pulses monthly 1, 2

Disease-Specific Modifications

  • For Ph-positive ALL, the National Comprehensive Cancer Network recommends adding a tyrosine kinase inhibitor, such as imatinib 600 mg daily or dasatinib 140 mg daily, starting day 1 of cycle 1 1
  • For elderly patients (≥60 years), the National Comprehensive Cancer Network recommends dose reductions of 25-33% in cyclophosphamide and doxorubicin doses 7

Supportive Care Requirements

  • The National Comprehensive Cancer Network recommends G-CSF 5 mcg/kg SC daily starting 24 hours after chemotherapy completion until ANC >1,000/μL 4
  • Mandatory hepatitis B screening with HBsAg and anti-HBc before treatment initiation is recommended 3, 5

Response Assessment

  • The National Comprehensive Cancer Network defines complete remission as <5% blasts with trilineage hematopoiesis in the bone marrow, ANC >1,000/μL, platelets >100,000/μL, and complete resolution of extramedullary disease 2

Expected Outcomes

  • The complete remission rate for acute lymphoblastic leukemia is 81-92% 7
  • The 5-year overall survival for acute lymphoblastic leukemia is 38-52% 7
  • The median overall survival for acute lymphoblastic leukemia is 32 months 7
