Promethazine Intravenous Administration Guidelines

Dosage and Infusion Rate

  • Infuse promethazine at a rate not exceeding 25 mg per minute to reduce the risk of hypotension. This recommendation follows standard safety protocols for IV promethazine administration. 1

  • Typical IV dosing ranges from 12.5 mg to 25 mg per single dose, with cumulative doses of 25 mg to 100 mg when used as an adjunct to other sedatives. These dosing parameters are drawn from clinical pharmacology references. [2][3]

Pharmacodynamics

  • Onset of therapeutic effect occurs within approximately 5 minutes after IV injection, and the effect lasts for 4–6 hours. This time course applies to standard adult dosing regimens. [1][2]3

Tissue Safety

  • IV promethazine can cause thrombophlebitis, tissue necrosis, and gangrene if extravasation occurs, emphasizing the need for careful administration technique. These adverse‑tissue effects are well documented. 1

  • Repeated or prolonged courses of IV promethazine are discouraged because of the heightened risk of serious tissue injury. Clinical guidance advises limiting duration of IV therapy. 1
