Management of Abdominal Pain in Patients Taking Semaglutide

Diagnostic Approach

  • The American College of Gastroenterology recommends performing laboratory workup including complete blood count (CBC), comprehensive metabolic panel (CMP), and serum amylase/lipase to evaluate for pancreatitis in patients taking semaglutide who develop abdominal pain 1
  • Consider abdominal imaging (CT scan) to rule out serious complications such as pancreatitis, bowel perforation, portal vein thrombosis, or ischemic colitis in patients taking semaglutide who develop abdominal pain 1

Management Algorithm

  • For patients with mild symptoms without concerning features, the European Society of Gastroenterology recommends temporary discontinuation of semaglutide and monitoring for symptom improvement, with supportive care including hydration and dietary modifications 1, 2

Treatment Considerations

  • For confirmed pancreatitis, the American Gastroenterological Association recommends discontinuing semaglutide permanently and managing according to standard pancreatitis protocols (NPO, IV hydration, pain management) 1
  • For other GI symptoms without pancreatitis, the British Society of Gastroenterology recommends considering antispasmodics for pain management, and tricyclic antidepressants (starting at low doses) may help with visceral pain 2, 3, 4

Prevention and Long-term Management

  • When restarting GLP-1 receptor agonists after resolution, the American Diabetes Association recommends considering a lower dose or different medication in the same class, with close monitoring for recurrence of symptoms 2
