Methylphenidate and REM Sleep Behavior Disorder

Understanding RBD and Its Known Causes

  • RBD is characterized by dream enactment behaviors during REM sleep due to loss of normal muscle atonia, complex motor behaviors associated with dream content, and potential for self-injury or injury to bed partners, as noted by the American Academy of Sleep Medicine and the American Geriatrics Society 1, 2
  • Known causes and associations of RBD include neurodegenerative disorders, particularly α-synucleinopathies, such as Parkinson's disease, dementia with Lewy bodies, and multiple system atrophy, according to the American Academy of Sleep Medicine 1, 3
  • Certain medications, including antidepressants and β-blockers, are associated with RBD, as documented by the American Academy of Sleep Medicine and the American Geriatrics Society 1, 2, 4

Diagnostic Considerations

  • The American Academy of Sleep Medicine recommends that RBD diagnosis requires specific diagnostic criteria, including REM sleep without atonia on polysomnography, and that other causes of abnormal sleep behaviors should be ruled out, such as non-REM parasomnias, sleep apnea, periodic limb movements of sleep, and nocturnal seizures 1, 2

Clinical Implications

  • If a patient on methylphenidate develops symptoms suggestive of RBD, the American Academy of Sleep Medicine suggests that polysomnography is essential for diagnosis to confirm REM sleep without atonia, and that other established medication causes of RBD and underlying neurodegenerative disorders should be considered 1, 2, 3
  • The American Geriatrics Society notes that treatment options for RBD include clonazepam as the first-line pharmacological treatment, and environmental safety measures to prevent injury 2, 5
