Assessment of Hydration Status

Clinical Guidelines

  • The European Society for Clinical Nutrition and Metabolism (ESPEN) guidelines state that urine specific gravity shall not be used to assess hydration status in older adults due to poor diagnostic accuracy 1, 2
  • Serum osmolality greater than 300 mOsm/kg is the gold standard for diagnosing dehydration, not urine markers, as recommended by the ESPEN guidelines 1, 2, 3

Diagnostic Considerations

  • Certain clinical conditions, such as uncontrolled diabetes mellitus, nephrotic syndrome, and after radiocontrast administration, can distort the specific gravity-osmolality relationship, making urine specific gravity an unreliable indicator of hydration status 1, 2
