Diagnostic Interpretation and Management of Mantle Cell Lymphoma

Key Diagnostic Features

  • The National Comprehensive Cancer Network recommends that the immunophenotypic profile of CD5+ (dim), CD23-, FMC7+, CD200- is the classic signature that distinguishes MCL from chronic lymphocytic leukemia (CLL) 1, 2
  • CD23 negativity is the most important discriminator, as CLL/SLL characteristically expresses CD23, while MCL does not 5, 3, 4
  • CD200 negativity strongly argues against CLL and favors MCL, as CLL is typically CD200+ 3, 4
  • Dim CD5 expression is characteristic of MCL 1, 2

Mandatory Confirmatory Testing

  • The National Comprehensive Cancer Network recommends cyclin D1 expression by flow cytometry or immunohistochemistry for MCL diagnosis 1, 2, 3, 4
  • FISH for t(11;14)(q13;q32) is the hallmark translocation of MCL 1, 2, 3, 4
  • SOX11 immunohistochemistry may be used if cyclin D1 is negative but clinical suspicion remains high, to exclude rare cyclin D1-negative MCL 4

Additional Diagnostic Workup Required

  • Lymph node or bone marrow biopsy is essential for definitive diagnosis and to assess morphologic variant (classic vs. blastoid/pleomorphic) 1, 2
  • Complete blood count with differential should be performed to assess absolute lymphocyte count and cytopenias 3
  • CT chest/abdomen/pelvis should be performed to evaluate lymphadenopathy, splenomegaly, and hepatomegaly 3
  • Bone marrow aspirate and biopsy with flow cytometry and FISH should be performed 3
  • Ki-67 proliferation index is a critical prognostic marker in MCL 1, 2

Treatment Implications

  • The National Comprehensive Cancer Network recommends that MCL is an aggressive lymphoma requiring prompt treatment in most cases, unlike CLL which can often be observed 1, 2
  • Observation is rarely appropriate for MCL, even in asymptomatic patients, given its aggressive natural history 1, 2
  • Standard induction regimens include intensive chemotherapy protocols, such as R-HyperCVAD, Bendamustine-rituximab, and VR-CAP 1
  • Consolidation with autologous stem cell transplant should be considered in younger, fit patients achieving remission 1
  • Maintenance rituximab improves progression-free survival 1
