Nebulized β‑Agonist + Ipratropium Therapy in Patients with Chronic Kidney Disease

Dosage and Combination Recommendations

  • The British Thoracic Society recommends that acute exacerbations be treated with a combined nebulized regimen of 2.5–10 mg β‑agonist together with 250–500 µg ipratropium bromide. 1, 2, 3
  • For CKD patients experiencing an acute cough exacerbation, the advised dose per nebulization is salbutamol 2.5–5 mg plus ipratropium 250–500 µg. 1, 2

Frequency and Duration of Treatment

  • Nebulizations should be administered every 4–6 hours for a total of 24–48 hours, or until the patient shows clinical improvement. 1, 2

Nebulizer Gas Choice in CKD with CO₂ Retention

  • When the patient has carbon‑dioxide retention, the nebulizer must be driven by room air rather than high‑flow oxygen to avoid aggravating hypercapnia. 1, 2, 3

Fluid Considerations for the Saline Diluent

  • Adding 5 cc of preservative‑free normal saline to the nebulizer contributes only minimal systemic fluid absorption; this volume is clinically insignificant even for CKD patients on strict fluid restriction. 2
