Switching from Haloperidol to Risperidone
Initial Risperidone Dosing
- Start risperidone at 2 mg daily while continuing haloperidol 15 mg, providing adequate dopamine blockade to prevent symptom breakthrough in patients switching from haloperidol to risperidone 1, 2
- For elderly patients, consider starting risperidone at 0.5-1 mg daily with a maximum of 5 mg daily to minimize adverse effects 1, 2
Risperidone Titration After Haloperidol Discontinuation
- Increase risperidone by 1-2 mg every 3-7 days after haloperidol is fully discontinued, with a target dose range of 4-6 mg daily 1, 2
- The maximum recommended dose is 6 mg daily, as doses above this significantly increase extrapyramidal symptom risk 2
Critical Safety Monitoring
- Continue any existing anticholinergic medications during the transition, as both haloperidol and risperidone cause extrapyramidal symptoms 1
- Obtain baseline and follow-up QTc if haloperidol doses were high, as both medications prolong QTc interval 1
- Monitor for orthostatic hypotension, particularly in the first 2 weeks of risperidone initiation 1
Managing Breakthrough Agitation
- Use haloperidol 0.5-2 mg every 1-4 hours as needed for acute agitation, regardless of the transition status 3, 1
- Avoid increasing standing doses prematurely; allow 14-21 days at each dose level before determining inadequate response 4
- If agitation is refractory to neuroleptics, add lorazepam 0.5-2 mg every 4-6 hours rather than escalating antipsychotic doses 3
