Albendazole Dosing for Pulmonary Eosinophilia in Adults

Dosing Recommendations

  • The American Society of Infectious Diseases recommends albendazole 400 mg twice daily for 5-7 days for pulmonary eosinophilia in adults, with the specific duration depending on the suspected parasitic cause and severity of symptoms 1
  • For strongyloidiasis with pulmonary manifestations, ivermectin 200 μg/kg as a single dose is preferred first-line therapy, according to the Infectious Diseases Society of America 5
  • In cases of strongyloidiasis with pulmonary manifestations where ivermectin is unavailable, albendazole 400 mg twice daily for 7 days can be used as an alternative, as suggested by the Centers for Disease Control and Prevention 1

Diagnostic Approach

  • The American Society of Infectious Diseases recommends serology for common parasitic causes, such as Strongyloides and Toxocara, before treatment when possible 3
  • Stool microscopy and PCR for helminth detection are also recommended by the Infectious Diseases Society of America before treatment when possible 3
  • Sputum examination for larvae in severe cases is recommended by the American Thoracic Society 2

Precautions and Follow-up

  • The Centers for Disease Control and Prevention recommend excluding Loa loa in people who have traveled to endemic regions before treating with ivermectin 5, 1
  • The Infectious Diseases Society of America advises using caution with steroids in cases where strongyloidiasis is suspected, as they may precipitate hyperinfection syndrome 2
  • For schistosomiasis-related pulmonary eosinophilia, the World Health Organization recommends repeat treatment at 8 weeks as eggs and immature forms may be resistant to initial treatment 5
