Management of Elevated Bilirubin in Patients on Multiple Antimicrobials

Immediate Actions

  • The Centers for Disease Control and Prevention recommends discontinuing suspected hepatotoxic medications if bilirubin is significantly elevated (>5 times ULN) or if there are signs of severe liver dysfunction, prioritizing discontinuation of metronidazole and amphotericin B first 2

Monitoring

  • The American Association for the Study of Liver Diseases recommends checking comprehensive liver function tests including ALT, AST, ALP, GGT, total and direct bilirubin, and monitoring liver function every 2-3 days initially until improvement is noted 2, 3
  • The National Institute of Diabetes and Digestive and Kidney Diseases suggests assessing for other signs of liver dysfunction (coagulopathy, encephalopathy) and monitoring renal function, as many of these agents (particularly amikacin) are nephrotoxic 1

Alternative Antimicrobial Strategies

Resumption of Therapy

  • The British Thoracic Society recommends considering sequential reintroduction of essential medications one at a time, beginning with the most critical agent at a lower dose, monitoring liver function tests every 2-3 days, and allowing 2-3 days between introducing each medication to identify the specific causative agent if hepatotoxicity recurs 2

Renal Function

  • The Centers for Disease Control and Prevention suggests that amikacin requires close monitoring of renal function, as impaired renal clearance can lead to toxicity, and dosing frequency should be reduced in renal impairment 1
