Anticoagulation Management in Hemodialysis Patients with NSTEMI and AKI Stage 3

Primary Recommendation

  • The American College of Cardiology recommends UFH as the first-line anticoagulant for patients with NSTEMI and severe renal impairment (CrCl <30 mL/min or requiring hemodialysis), as it does not require renal dose adjustment and allows precise titration via aPTT monitoring 1, 2
  • The American College of Cardiology/American Heart Association guidelines state that for patients with ACS and renal insufficiency, bivalirudin or UFH may be considered, with no recommendation supporting enoxaparin in this context 1

Anticoagulant Considerations

  • The American College of Cardiology recommends against the use of fondaparinux in patients with CrCl <30 mL/min due to its contraindication in severe renal impairment 1, 2
  • Bivalirudin may be considered as an alternative to UFH in patients with NSTEMI and renal insufficiency, with a dosing regimen of 0.75 mg/kg bolus and 1.75 mg/kg/h IV infusion during PCI, reduced to 1 mg/kg/h if CrCl <30 mL/min 1

UFH Dosing and Monitoring

  • The initial loading dose of UFH is 60 IU/kg IV bolus (maximum 4000 IU), followed by a maintenance infusion of 12 IU/kg/h (maximum 1000 IU/h), adjusted to therapeutic aPTT range of 60-80 seconds 1
  • UFH does not require renal dose adjustment, making it ideal for HD patients, and allows precise titration via aPTT monitoring with a shorter half-life 1, 2, 4

Enoxaparin Use and Monitoring

  • If enoxaparin is absolutely necessary, the dose should be reduced to 1 mg/kg subcutaneously once daily for patients with CrCl <30 mL/min, with mandatory anti-Xa monitoring to prevent drug accumulation and bleeding complications 1, 2, 3
  • The target therapeutic anti-Xa range is 0.5-1.0 IU/mL for twice-daily dosing and >1.0 IU/mL for once-daily dosing, with peak anti-Xa levels checked 4 hours after administration, only after 3-4 doses have been given 2

Critical Pitfalls to Avoid

  • Switching between enoxaparin and UFH during the same hospitalization increases bleeding risk and should be avoided 3, 5
  • Standard twice-daily enoxaparin dosing (1 mg/kg q12h) should not be used in patients with CrCl <30 mL/min, and empiric enoxaparin dosing without anti-Xa monitoring in HD patients should be avoided if enoxaparin must be used 1, 2
