Management of Furosemide When Starting Eplerenone

Initial Assessment and Monitoring

  • The European Society of Cardiology recommends checking baseline renal function and serum electrolytes before initiating eplerenone, with a target potassium level of ≤5.0 mmol/L and creatinine <220 μmol/L (2.5 mg/dL) in men or <2.0 mg/dL in women 1, 2, 3
  • The American Heart Association suggests verifying estimated glomerular filtration rate (eGFR) is >30 mL/min/1.73 m² before starting eplerenone 2, 4

Furosemide Adjustment and Eplerenone Initiation

  • The American College of Cardiology recommends reducing furosemide dose by approximately 25-50% when initiating eplerenone to minimize risk of dehydration and electrolyte disturbances, with careful monitoring of renal function and electrolytes 2
  • The European Society of Cardiology advises starting eplerenone at 25 mg once daily (or 25 mg every other day if eGFR 30-49 mL/min/1.73 m²) and discontinuing potassium supplements due to its potassium-sparing effects 2

Management of Complications and Dose Titration

  • The European Heart Journal recommends that if potassium rises to >5.5 mmol/L, the eplerenone dose should be halved (to 25 mg on alternate days) and furosemide dose considered for increase, with a strength of evidence rated as moderate 1, 3
  • The American Heart Association suggests considering eplerenone dose up-titration after 4-8 weeks if potassium and renal function remain stable, with a target dose of 50 mg once daily 1, 3
