Differential Diagnosis for Pinpoint Pupils

Life-Threatening Causes

  • Pupils typically measure 1-2mm and may be difficult to visualize without magnification, which is a characteristic feature of opioid intoxication 1
  • Response to naloxone is both diagnostic and therapeutic for opioid overdose, indicating the presence of opioid toxicity 2

Physiologic Considerations

  • Pupils in older adults may measure 2-3mm even in dim lighting due to age-related miosis, which is a normal finding 3
  • Near reflex during accommodation causes physiologic pupillary constriction, which is a normal finding when examining patients focusing on near objects 3

Critical Assessment Approach

  • The American Academy of Ophthalmology suggests that when evaluating pinpoint pupils, immediately assess symmetry of pupils, as bilateral suggests toxic/metabolic cause or pontine pathology, and unilateral suggests Horner's syndrome 3
  • The American Academy of Pediatrics recommends assessing light reactivity, as preserved in most causes except Argyll Robertson pupils 1
  • Associated neurologic signs, including ptosis, extraocular movements, and extremity strength, should be evaluated to identify potential causes of pinpoint pupils 2, 5

Common Pitfalls

  • The American Academy of Ophthalmology warns against confusing anisocoria with abnormal pupillary light response, as these represent different pathophysiologic processes, with a strength of evidence level of high 4
