NPO Status for Stress Tests

General Recommendations for Stress Testing

  • The American College of Cardiology recommends stress testing as a common diagnostic procedure to evaluate cardiac function under physical exertion, which does not require fasting 1
  • Standard protocols like the Bruce treadmill protocol are designed to be performed without specific NPO requirements 2

Specific Considerations

  • A standard exercise stress test, which involves walking on a treadmill or using a stationary bicycle, monitoring of ECG, blood pressure, and heart rate, can be performed without fasting, according to the European Heart Journal and Mayo Clinic Proceedings 2, 3, 4
  • Stress tests with imaging, such as stress echocardiography, generally have no strict NPO requirement, but follow specific instructions from your provider, as recommended by the European Heart Journal and Circulation 3, 5

Practical Considerations

  • Maintaining adequate hydration before a stress test is important for optimal performance, as stated in Circulation 2
  • Regular medications should generally be taken as prescribed unless specifically instructed otherwise by your healthcare provider, according to the Mayo Clinic Proceedings and the American College of Cardiology 4, 6

Special Considerations

  • For patients with diabetes or other conditions requiring regular food intake, it is recommended to discuss with your provider before the test, as advised by the Mayo Clinic Proceedings 4
