Management of Post-Paracentesis Hemodynamic Instability
Initial Evaluation and Medical Treatment
- The American Association for the Study of Liver Diseases recommends initial management of post-paracentesis hemodynamic instability with fluid resuscitation (intravenous albumin), transfusion of blood products if bleeding occurs, and correction of coagulation disorders; if instability persists despite medical treatment, transcateter embolization with coil or laparoscopic ligation of the bleeding vessel should be considered 1
- Hemorrhage occurs in 0.2-2.2% of procedures, with a mortality rate of 0.02%, and may manifest within the first 6-24 hours or up to 1 week after the procedure 1
- The European Association for the Study of the Liver states that disfunción circulatoria post-paracentesis (DCPP) results from the rapid decrease in intra-abdominal pressure, improving venous return and transiently increasing cardiac output, but subsequently decreasing effective arterial volume 2, 3, 4
Management of Post-Paracentesis Hemorrhage
- The initial treatment should be medical and aggressive, with fluid resuscitation using volume expanders immediately 1
- Transfusion of blood products, such as packed red blood cells, should be performed as needed to maintain adequate hemoglobin levels 1
- Correction of coagulopathy with fresh frozen plasma and/or platelets should be administered if severe coagulation disorders are present 1
Management of Disfunción Circulatoria Post-Paracentesis
- The administration of intravenous albumin (6-8 g per liter of ascites removed) is recommended when more than 5 liters of ascites are removed, as it reduces the incidence of DCPP (18.5%) compared to dextran-70 (34.4%) or polygeline (37.8%) 5, 6, 7, 8
- Artificial volume expanders are associated with increased activation of the renin-angiotensin-aldosterone system 7, 2, 3
Interventions When Medical Treatment Fails
- If hemodynamic instability persists despite medical treatment, invasive interventions such as transcateter embolization with coil or laparoscopic ligation of the bleeding vessel should be considered 1
- In cases of severe bleeding, TIPS (transjugular intrahepatic portosystemic shunt) or liver transplantation may be considered 1
Monitoring and Follow-up
- The mean arterial pressure decreases by an average of <8 mmHg post-paracentesis, but may be greater in advanced liver disease 5, 6, 7
- Renal function, including serum creatinine and electrolytes, should be monitored, especially sodium levels 5, 6, 7
- Central venous pressure is useful for detecting intravascular volume depletion 8
- Advanced liver disease, absence of peripheral edema, and pre-existing renal insufficiency are risk factors for severe hypotension 1, 5, 6, 7
