Atrial Fibrillation Risk Associated with Levofloxacin

Mechanism and Risk

  • Fluoroquinolones, including levofloxacin, are associated with cardiac effects, and can induce drug-induced atrial fibrillation (DIAF) 1, 2

Risk Factors for Levofloxacin-Induced Atrial Fibrillation

  • Patients with older age (>65 years), pre-existing cardiovascular disease, history of cardiac arrhythmias, left atrial enlargement, hypertension, and heart failure are at higher risk for developing atrial fibrillation with levofloxacin 2

Management Recommendations

  • If atrial fibrillation develops during levofloxacin therapy, discontinue levofloxacin immediately if possible, and consider alternative antibiotics that don't prolong QT interval 1, 3
  • Monitor ECG in high-risk patients, particularly at baseline and after adding any new medication known to prolong QT 4
  • Manage atrial fibrillation according to standard guidelines with rate or rhythm control strategies as appropriate 3, 5

Clinical Pitfalls to Avoid

  • Failing to recognize atrial fibrillation as a potential adverse effect of levofloxacin therapy, and not considering drug-induced atrial fibrillation in the differential diagnosis of new-onset AF 1, 2
