Olanzapine Dosing and Administration Guidelines

Special Populations

  • For elderly or debilitated patients, a lower starting dose of 2.5-5 mg is recommended by the American College of Clinical Pharmacology, due to increased risk of adverse effects 1
  • In patients with hepatic impairment, a starting dose of 2.5 mg daily is suggested by the National Institute of Mental Health, to minimize risk of toxicity 1

Monitoring and Safety Considerations

  • When combining olanzapine with benzodiazepines, caution is advised due to the risk of oversedation and respiratory depression, as reported by the American Society of Health-System Pharmacists 2, 1
  • Fatalities have been reported with concurrent use of benzodiazepines and high-dose olanzapine, highlighting the need for careful dose monitoring and management, as warned by the FDA 2, 1
