Ocular Trauma Classification and Assessment

Clinical Assessment Framework

  • The American Academy of Ophthalmology recommends pupillary examination, intraocular pressure measurement, and exophthalmometry as part of the initial evaluation components for ocular trauma, with forced duction testing helping to distinguish restriction from paresis of extraocular muscles 1, 2
  • The American College of Radiology suggests non-contrast thin-section orbital CT with multiplanar reconstructions as the imaging modality of choice for orbital trauma, with a sensitivity of 94.9% for identifying orbital fractures, displaced fragments, and metallic foreign bodies 3, 5

Imaging Recommendations

  • The American College of Radiology advises that MRI is absolutely contraindicated if metallic foreign bodies are suspected due to potential movement of the object 3, 5
