Standard Management for Locally Advanced Pancreatic Adenocarcinoma
First-Line Treatment Protocol and Dosing
- The European Society for Medical Oncology recommends gemcitabine monotherapy at a dose of 1000 mg/m² administered over 30 minutes, weekly for 7 weeks followed by 1 week rest, then weekly for 3 weeks followed by 1 week rest in subsequent cycles for patients with locally advanced pancreatic adenocarcinoma 1, 2
- For patients ≤75 years with good performance status (0-1) and bilirubin ≤1.5 ULN, the National Comprehensive Cancer Network recommends FOLFIRINOX, which consists of 5-FU: 400 mg/m² bolus, then 2400 mg/m² over 46 hours, Leucovorin: 400 mg/m², Irinotecan: 180 mg/m², and Oxaliplatin: 85 mg/m², administered every 2 weeks 4, 6
Second-Line Treatment Protocol and Dosing
- After first-line gemcitabine failure, the National Comprehensive Cancer Network recommends 5-FU/Leucovorin plus Oxaliplatin (OFF regimen), which consists of 5-FU: 2000 mg/m² (24-hour infusion), Leucovorin: 200 mg/m², and Oxaliplatin: 85 mg/m², administered every 2 weeks 8, 7
- Alternatively, nanoliposomal irinotecan with 5-FU/Leucovorin can be considered, which consists of nanoliposomal irinotecan: 70 mg/m², 5-FU: 2400 mg/m² (46-hour infusion), and Leucovorin: 400 mg/m², administered every 2 weeks 7, 6
Palliative Management
- The European Society for Medical Oncology recommends endoscopic placement of a metallic biliary stent for biliary obstruction 4, 2
- Opioids, such as morphine, should be administered for severe pain 4
- Duodenal obstruction can be managed with an expandable metal stent 4, 2
- Hypofractionated radiotherapy may improve pain control, and percutaneous or EUS-guided celiac plexus blockade can be considered for patients with poor tolerance to opioid analgesics 4, 2
