Oral Potassium Replacement Recommendations for Serum Potassium ≈ 2.6 mEq/L

Dosage and Formulation

  • For patients with a serum potassium of approximately 2.6 mEq/L, initiate oral potassium chloride at a total daily dose of 40–80 mEq, divided into multiple administrations, and preferentially use an immediate‑release liquid formulation when available to achieve faster absorption and more predictable serum‑potassium rise. 1

Preferred Route of Administration

  • When the gastrointestinal tract is functional and the serum potassium exceeds 2.5 mEq/L, oral potassium replacement is the preferred route over intravenous therapy. 1

Administration Strategy to Minimize Adverse Effects

  • To reduce gastrointestinal irritation and improve tolerance, the total daily oral potassium dose should be split into three to four separate doses rather than given as a single large bolus. 1
