Doxycycline Dosing Guidelines
Introduction to Doxycycline Dosing
- The American Academy of Pediatrics recommends doxycycline 100 mg twice daily for serious infections in patients ≥8 years old 2
- For patients ≥8 years old, doxycycline is FDA-approved with standard dosing of 100 mg twice daily for serious infections 2
Weight-Based Dosing
- For a 50 kg patient, the recommended dosing is 200 mg total (100 mg every 12 hours) on day 1, followed by 100 mg daily maintenance dosing for most indications 1
- The maintenance dose can be given as a single 100 mg dose or divided into 50 mg every 12 hours 1
Age Considerations
- For patients ≥8 years old, doxycycline is FDA-approved with standard dosing of 100 mg twice daily for serious infections 2
- For patients <8 years old, doxycycline can still be used when benefits outweigh risks, particularly for life-threatening infections, as short-term use (<21 days) does not cause permanent tooth staining 1
Condition-Specific Adjustments
- For Tickborne Rickettsial Diseases (RMSF, Ehrlichiosis), the recommended dose is 100 mg every 12 hours for a minimum of 5-7 days total, continuing at least 3 days after fever resolves 1
- For Chlamydia/Gonorrhea, the recommended dose is 100 mg twice daily for 7 days 5
- For Anthrax Post-Exposure Prophylaxis, the recommended dose is 100 mg twice daily for 60 days 1
Critical Administration Guidelines
- Patients should take doxycycline with a full glass of water and remain upright for 30 minutes to prevent esophageal irritation and ulceration 1
- Patients should avoid taking antacids containing aluminum, calcium, or magnesium, iron supplements, and dairy products (if taking on empty stomach for maximum absorption) within 2-3 hours of administration 1, 4, 5
Common Pitfalls to Avoid
- Doxycycline has a superior safety profile compared to tetracycline, minocycline, or oxytetracycline, which remain contraindicated in young children 4
- Prolonged or repeated courses of doxycycline should be avoided in children <8 years 4
