Zinc Supplementation Guidelines for Adults with Low Body Mass
Standard Dosing Framework
- The American College of Nutrition recommends 0.5-1 mg/kg per day of elemental zinc given orally for 3-4 months if acquired zinc deficiency is present, with organic zinc compounds preferred over zinc sulfate due to better tolerability 1
- For adults with low body mass index, the recommended dose is 0.5-1 mg/kg per day of elemental zinc, which translates to approximately 25-50 mg elemental zinc daily for a 50 kg adult 3, 1
- Organic zinc compounds, such as zinc gluconate, zinc histidinate, and zinc orotate, demonstrate better tolerability than inorganic zinc sulfate and zinc chloride 3, 1, 2
Important Dosing Considerations for Low Body Mass Patients
- Weight-based dosing (0.5-1 mg/kg) is particularly relevant for adults with low body mass, as it prevents both under- and over-dosing 1
- A 45 kg adult would require 22.5-45 mg elemental zinc daily for deficiency treatment, and a 60 kg adult would require 30-60 mg elemental zinc daily for deficiency treatment 2
Monitoring Requirements
- Measure plasma zinc levels to confirm clinical zinc deficiency, and simultaneously determine CRP and albumin for proper interpretation of zinc levels 3, 1
- Recheck zinc levels after 3 months of supplementation, and monitor copper levels concurrently, as zinc supplementation can induce copper deficiency 4, 2
- Maintain a zinc-to-copper ratio of 8-15 mg zinc to 1 mg copper when providing both supplements 4, 2
Special Circumstances in Low BMI Patients
- If the patient has gastrointestinal losses, IV zinc up to 12 mg per day may be required 3, 1
- Patients with eating disorders are at increased risk of zinc deficiency and may require the higher end of the dosing range 3, 2
