GLP-1 Receptor Agonists in Type 2 Diabetes Management

Primary Recommendation

  • The American Diabetes Association recommends GLP-1 receptor agonists as the first injectable glucose-lowering therapy for type 2 diabetes when oral agents fail to achieve glycemic targets, prioritized over insulin due to superior cardiovascular and renal protection, weight loss benefits, and minimal hypoglycemia risk 1, 2

Mechanism of Action

  • GLP-1 receptor agonists work through multiple complementary pathways, including glucose-dependent insulin secretion enhancement, glucagon suppression, delayed gastric emptying, and central appetite suppression 3, 4

Clinical Efficacy

  • GLP-1 receptor agonists reduce major adverse cardiovascular events (MACE) including myocardial infarction, stroke, and cardiovascular death in patients with established atherosclerotic disease 3, 5
  • Liraglutide, lixisenatide, exenatide weekly, semaglutide, dulaglutide, and albiglutide have demonstrated cardiovascular benefits 3, 5
  • GLP-1 receptor agonists reduce albuminuria and slow eGFR decline 5

Clinical Positioning

  • The American Diabetes Association recommends initiating GLP-1 receptor agonists as the first injectable therapy when metformin ± other oral agents fail to achieve HbA1c targets within 3 months 1, 2
  • GLP-1 receptor agonists are preferred over insulin in patients needing injectable therapy, unless extreme hyperglycemia (HbA1c >9% or FPG ≥11.1 mmol/L) with symptoms is present 1, 2

Renal Dosing Considerations

  • Dulaglutide, liraglutide, semaglutide require no dose adjustment for eGFR 30-59 mL/min/1.73m² 5
  • Lixisenatide requires no dose adjustment for eGFR 30-59 mL/min/1.73m² 5

Adverse Effects and Management

  • GLP-1 receptor agonists have an intrinsically low risk of hypoglycemia due to glucose-dependent mechanism 4
  • The risk of hypoglycemia increases when combined with sulfonylureas or insulin - reduce doses of these agents when initiating GLP-1 agonists 2

Contraindications

  • Type 1 diabetes is an absolute contraindication for GLP-1 receptor agonists 2

Drug Interactions

  • GLP-1 receptor agonists can delay gastric emptying, reducing absorption of oral drugs 8
  • Monitor prothrombin time more frequently after initiating or changing GLP-1 dose when taking warfarin 8
