Emergency Department Management of Esophageal Foreign Bodies
Initial Assessment
- The American Academy of Pediatrics recommends that children aged 3-14 years who ingest foreign bodies, such as bottle tops, should be assessed for signs of complete obstruction, including drooling, inability to handle secretions, and respiratory distress 3
- A detailed history regarding the time of ingestion, type of bottle top, and symptoms should be obtained to guide further management 1
- Evaluation of vital signs, particularly respiratory status, is crucial as foreign bodies can potentially dislodge and cause airway compromise 3
Diagnostic Approach
- Biplanar radiography (anteroposterior and lateral views) should be performed as the initial imaging study to confirm the presence and location of the foreign body, and to distinguish between esophageal and tracheal location 2
- The American College of Radiology recommends that biplanar radiography be used to identify the exact anatomical level of impaction 1
- If the foreign body is not radiopaque, additional imaging such as contrast studies or CT scan may be considered 4
Management Algorithm
- For symptomatic patients with complete obstruction, emergent endoscopic removal should be performed within 2-6 hours 1
- Rigid endoscopy is preferred for foreign bodies in the upper esophagus, while flexible endoscopy is the first-line approach for most esophageal foreign bodies 2
- For asymptomatic patients, urgent endoscopy within 24 hours is recommended if the foreign body is in the esophagus, and conservative management with serial examinations and follow-up radiographs may be considered if the foreign body has passed into the stomach 1
Potential Complications to Monitor
- Prolonged retention of foreign bodies in the esophagus can lead to pressure necrosis, perforation, and mediastinitis 2
- Aspiration risk is a concern if the object dislodges and enters the airway 1
- The smaller diameter of a child's airway means that even small changes in the cross-section can lead to dramatic changes in airway resistance and air flow 3
