Antiemetic Recommendations for Children

Context-Specific Recommendations

  • The American Academy of Pediatrics recommends weight-based dosing of ondansetron for optimal efficacy and safety in children with gastroenteritis-associated vomiting 1, 2
  • The Infectious Diseases Society of America recommends ondansetron for children over 4 years, though it is commonly used in younger children off-label with evidence support 1
  • Ondansetron facilitates oral rehydration therapy, which remains the mainstay of gastroenteritis treatment, and should be used alongside rehydration efforts 1, 2
  • For moderate-emetic-risk chemotherapy, the first choice is a 5-HT3 receptor antagonist (ondansetron or granisetron) plus dexamethasone 3, 4
  • For high-emetic-risk chemotherapy, the preferred regimen is a three-drug combination of 5-HT3 antagonist, dexamethasone, and aprepitant 6
  • For low-emetic-risk chemotherapy, ondansetron or granisetron alone is recommended 3, 5

Safety Considerations

  • The American Academy of Pediatrics recommends exercising special caution in children with heart disease due to QT interval prolongation risk when using ondansetron 1
  • In severe hepatic impairment, the recommended dose of ondansetron should not exceed 8 mg total daily dose, and hepatotoxicity should be monitored if multiple medications are being used 2
  • Adequate hydration should be ensured before or during ondansetron administration, and electrolytes, particularly potassium and magnesium, should be monitored 1, 2
