Fluticasone Dosing Strategy for Allergic Rhinitis

Evidence-Based Recommendations

  • The American College of Physicians recommends intranasal corticosteroids as first-line monotherapy for allergic rhinitis, with high-quality evidence supporting their superiority over oral antihistamines 2
  • Combination therapy with an intranasal antihistamine (azelastine) provides more than 40% greater symptom reduction than fluticasone alone, and should be considered if symptoms persist after 2 weeks 1, 2
  • Proper administration technique, such as directing the spray away from the nasal septum and using the contralateral hand, reduces the risk of epistaxis by four-fold 2, 3

Safety and Efficacy

  • The onset of action of fluticasone occurs within 12-24 hours, but maximal efficacy requires days to weeks of regular use 3
