Loperamide Dosage and Safety Guidelines

Dosing Recommendations

  • The maximum daily dose of loperamide for adults is 16 mg per day, as recommended by the American College of Physicians, to balance efficacy and safety 1
  • Start with 4 mg (two 2 mg capsules) as the initial dose, and follow with 2 mg after each unformed stool or every 2-4 hours, according to the American Gastroenterological Association 1
  • Do not exceed 16 mg (eight 2 mg capsules) in 24 hours, as advised by the American College of Physicians, to minimize the risk of adverse effects 1, 2, 3, 4, 5, 6, 7, 8

Contraindications and Precautions

  • Do not use loperamide in grade 3-4 diarrhea or severe immunotherapy-induced colitis, as recommended by the National Comprehensive Cancer Network 1, 9, 10
  • Avoid using loperamide in combination with anticholinergic and opioid agents when paralytic ileus is a concern, as cautioned by the American Society of Clinical Oncology 7

Monitoring and Special Populations

  • Patients on high-dose loperamide (approaching 16 mg daily) should be monitored for paralytic ileus, though this complication is rare, as noted by the American College of Gastroenterology 1, 9, 10
