Cephalexin Use in Patients with Sulfonamide Antibiotic Allergy

Chemical Distinction

  • Cephalexin is a first‑generation cephalosporin that contains a β‑lactam ring, a structure that is chemically unrelated to the sulfonamide moiety of antibiotics such as trimethoprim‑sulfamethoxazole, indicating inherent lack of cross‑reactivity. 1, 2

Evidence of No Cross‑Reactivity

  • Comparative molecular analyses have demonstrated that the structural dissimilarity between cephalosporins and sulfonamide antibiotics prevents immunologic cross‑reactivity in patients allergic to sulfonamides. 2

Clinical Recommendations

  • For patients with a documented sulfonamide antibiotic allergy, cephalexin may be prescribed at full therapeutic doses without the need for special precautions, allergy testing, graded challenges, or allergy‑alert overrides; standard clinical monitoring is sufficient. 1
