Septrin Prophylactic Dosing Guidelines

Clinical Context Considerations

  • For peritoneal dialysis patients with BSA 1.1 m², the BSA-based dosing approach is well-established in nephrology practice, as recommended by the American Society of Nephrology 1, 2, 3, 4

Dosing Recommendations

  • The American Academy of Pediatrics recommends a prophylactic dose of 750 mg/m²/day sulfamethoxazole with 150 mg/m²/day trimethoprim given in equally divided doses twice daily for pediatric patients 1
  • The Centers for Disease Control and Prevention recommends a standard adult prophylactic dose of one double-strength tablet (800mg/160mg) once daily for PCP prophylaxis in adults 1

Important Caveats

  • The National Institute of Diabetes and Digestive and Kidney Diseases recommends renal impairment requires dose adjustment: For creatinine clearance 15-30 mL/min, use half the usual regimen; for creatinine clearance below 15 mL/min, use is not recommended 1
  • The World Health Organization recommends monitoring for adverse effects: Approximately 25-33% of patients experience adverse effects, though treatment discontinuation is rarely necessary 1
