Long‑Term Monitoring and Management of Patients on Symtuza (Darunavir/Cobicistat/Emtricitabine/Tenofovir Alafenamide)

Virologic Monitoring

  • The British HIV Association recommends measuring HIV‑1 RNA every 6–12 months in patients on protease‑inhibitor‑based regimens who have achieved and maintained virologic suppression. Strength: guideline recommendation (high). 1
  • The WHO advises a 12‑month interval for HIV‑1 RNA testing after initial suppression has been confirmed. Strength: guideline recommendation (high). 1
  • Once sustained suppression is established, viral load testing can be performed at 6–12 month intervals. Strength: expert consensus (moderate). 1

Immunologic (CD4) Monitoring

  • CD4 count monitoring may be discontinued in patients with continuous viral suppression and CD4 counts consistently >250–300 cells/µL for at least one year. Strength: expert consensus (moderate). 1
  • A meta‑analysis showed that only 0.4 % of patients with sustained viral suppression experienced a confirmed CD4 decline. Strength: meta‑analysis (high). 1
  • Among patients with CD4 > 300 cells/µL and HIV‑RNA < 200 copies/mL, 97–99 % maintained CD4 > 200 cells/µL for at least four years. Strength: cohort data (moderate). 1

Hematologic Monitoring

  • Routine complete blood count (CBC) testing is not required for contemporary first‑line regimens that include Symtuza. Strength: guideline/consensus (moderate). 1
  • Phase 3 clinical trials demonstrated no hematologic toxicity associated with darunavir/cobicistat/emtricitabine/tenofovir alafenamide during long‑term follow‑up. Strength: randomized controlled trial (high). 1
  • CBC should be obtained only when clinically indicated by symptoms or comorbid conditions. Strength: expert opinion (low‑moderate). 1

Hepatitis B Co‑infection Monitoring

  • In patients co‑infected with hepatitis B virus, hepatitis B viral load and liver function tests should be monitored every 3–6 months. Strength: guideline recommendation (high). 2
  • Symtuza contains emtricitabine and tenofovir alafenamide, both of which have activity against hepatitis B virus. Strength: pharmacologic data (high). 2

Pregnancy Considerations

  • Symtuza should not be used during pregnancy because cobicistat‑containing regimens provide inadequate drug levels for effective viral suppression. Strength: guideline/clinical warning (high). 2
