Treatment of Recurrent Bacterial Vaginosis
Introduction to Treatment Guidelines
- The American College of Physicians recommends that male partners of women with recurrent bacterial vaginosis (BV) should be treated with combination oral metronidazole and topical clindamycin cream to reduce recurrence rates 1
Treatment Protocol
- The Centers for Disease Control and Prevention (CDC) suggests metronidazole 500 mg orally twice daily for 7 days as first-line treatment for female partners with recurrent BV 2
- For male partners, the recommended treatment is metronidazole 400 mg orally twice daily for 7 days, plus 2% clindamycin cream applied topically to penile skin twice daily for 7 days 2
Patient Instructions
- Patients should avoid all alcohol during metronidazole treatment and for 24 hours after completion to prevent disulfiram-like reactions 2, 3
- Refrain from unprotected intercourse for at least 14 days to allow treatment to take effect 2
Diagnosis and Differential Diagnosis
- Bacterial vaginosis (BV) can be distinguished from vulvovaginal candidiasis by pH level, odor, and discharge characteristics: BV is characterized by a pH >4.5, fishy odor, and thin discharge, while candidiasis has a pH ≤4.5, white/lumpy discharge, and itching 4
