Imipenem‑Cilastatin Dosing Recommendations and Key Clinical Considerations

Standard Dosing for Adults with Normal Renal Function

  • For patients with creatinine clearance ≥ 90 mL/min, the recommended regimen for susceptible organisms is 500 mg administered every 6 hours or 1000 mg administered every 8 hours, with a maximum total daily dose of 4 g. 1

Loading Dose Strategy in Critically Ill Patients

  • A loading dose equal to the intended maintenance dose (e.g., 500 mg or 1000 mg) should be given before starting intermittent maintenance therapy in critically ill patients, regardless of renal function, to compensate for the increased volume of distribution seen in sepsis. 2

Limitations of Prolonged Continuous Infusion

  • Imipenem‑cilastatin is not suitable for prolonged continuous infusion because it is chemically unstable; in 0.9 % NaCl at 25 °C the drug remains stable for only 2–3 hours. 2
  • Consequently, the drug must be administered in divided intermittent doses that are prepared immediately before infusion. 2
