Guideline-Directed Medical Therapy for ESRD Patients on Hemodialysis

Vascular Access Management

  • The American Journal of Kidney Diseases recommends arteriovenous fistulas (AVF) or arteriovenous grafts (AVG) over tunneled central venous catheters (CVC) for vascular access in ESRD patients on hemodialysis 2
  • For patients using AVF with intensive hemodialysis, the American Journal of Kidney Diseases recommends rope-ladder cannulation technique over buttonhole cannulation unless topical antimicrobial prophylaxis is used 1
  • If buttonhole cannulation technique is used, the American Journal of Kidney Diseases suggests applying mupirocin antibacterial cream to reduce infection risk 3
  • For patients requiring CVC access, the American Journal of Kidney Diseases recommends using "closed connector" devices over standard care to reduce complications 4

Dialysate Composition Management

  • The American Journal of Kidney Diseases recommends maintaining dialysate calcium at 1.50 mmol/L or higher for patients on long or long-frequent hemodialysis to ensure neutral or positive calcium balance 5
  • This approach helps avoid predialysis hypercalcemia and prevents oversuppression of parathyroid hormone (PTH) 5
  • If hypophosphatemia persists despite discontinuation of phosphate binders and diet liberalization, the American Journal of Kidney Diseases suggests using phosphate dialysate additives to maintain predialysis phosphate in the normal range 1

Common Pitfalls and Caveats

  • The evidence supporting Guideline-Directed Medical Therapy (GDMT) in ESRD is generally of very low quality, with most recommendations being conditional rather than strong 1, 2
