Management of Multiple System Atrophy
Diagnostic Approach
- Brain MRI without IV contrast is the preferred initial imaging modality for patients with cerebellar ataxia, looking for morphologic changes and signal alterations primarily involving the cerebellum and brainstem, with advanced MRI techniques like diffusion-weighted imaging helping to detect early changes, according to the American College of Radiology 1
MRI is the optimal imaging modality for differentiating MSA from PD, with characteristic findings including:
Finding Description Putaminal atrophy Significantly reduced putamen/caudate volume ratio (<1.6) Brainstem atrophy Particularly in the pons Infratentorial abnormalities Such as cerebellar ataxia "Hot cross bun" sign Specific to the MSA-C variant - Initial MRI evaluation should include assessment of putaminal atrophy and hypointensity, brainstem and cerebellar structures, and calculation of putamen/caudate volume ratio, with a ratio <1.6 suggesting MSA 2
- Multimodal MRI combining structural, iron-sensitive, and diffusion tensor imaging can improve diagnostic accuracy in uncertain cases, and DAT scan can confirm parkinsonism but not differentiate between MSA and PD 2
- Functional imaging can be used to distinguish Parkinson's disease from extrapyramidal side effects of medications, as part of a comprehensive diagnostic workup 3
- A DAT scan can effectively differentiate between idiopathic Parkinson's disease and drug-induced parkinsonism by showing abnormal patterns of dopaminergic depletion in idiopathic Parkinson's disease, but remaining normal in drug-induced parkinsonism, as supported by the American College of Radiology 2
Symptomatic Treatment
- Early integration of palliative care is recommended, focusing on quality of life, symptom management, and caregiver support, with advance care planning discussions initiated early in the disease course, as suggested by the Journal of Neurology, Neurosurgery and Psychiatry 4
- Regular neurological assessments are necessary to monitor disease progression, with MRI brain used to track structural changes, although it has limited value for routine follow-up, as noted by Nature Reviews Neurology 5
- Regular neurological assessments are necessary for monitoring disease progression and treatment response, as recommended by the American Academy of Neurology 3
- The American Psychiatric Association recommends treating acute dystonia with anticholinergic medication, and also suggests considering lowering antipsychotic dose, switching to another antipsychotic, or treating with anticholinergic medication for parkinsonism associated with antipsychotics, and considering lowering antipsychotic dose, switching to another antipsychotic, adding a benzodiazepine, or adding a beta-blocker for akathisia 6
Multidisciplinary Care
- Coordinate care between a neurologist, physical therapist, occupational therapist, speech therapist, urologist, pulmonologist, and palliative care specialist to provide comprehensive management for patients with MSA-C 1
- A multidisciplinary approach to Parkinson's disease care is strongly supported, with coordination of clinical care crucial for the management of complex neurological conditions, and should include specialists with expertise in movement disorders, particularly neurologists, according to the American College of Neurology 7
- Family Nurse Practitioners (FNPs) should not independently diagnose Parkinson's disease, but can play valuable roles in the care team, including screening and referral of patients with suspicious symptoms to neurologists or movement disorder specialists 3
- FNPs can participate in coordinated clinical care teams under specialist supervision, and provide ongoing management of stable Parkinson's patients after diagnosis by specialists, as well as medication monitoring and management in collaboration with specialists 3
