Causes of Elevated SHBG in a Healthy Man
Primary Causes of Elevated SHBG
- Elevated Sex Hormone-Binding Globulin (SHBG) in a healthy man can be caused by medications, aging, smoking, hyperthyroidism, hepatic disease, and HIV/AIDS 1, 2
- Medications such as anticonvulsants, estrogens, and thyroid hormone replacement can significantly increase SHBG levels 1, 2
- Hyperthyroidism, characterized by excess thyroid hormone, can cause elevated SHBG 1, 2
- Hepatic disease can impair SHBG metabolism, leading to increased circulating levels 1, 2, 3
- Aging is associated with naturally increasing SHBG levels in men 1, 2
- Smoking is linked to higher SHBG concentrations 1, 2
- HIV/AIDS infection can lead to elevated SHBG levels 1, 2
Physiological Mechanisms
- Elevated SHBG can lead to reduced free testosterone levels even when total testosterone appears normal 3
Clinical Significance
- Elevated SHBG can contribute to symptoms of hypogonadism even with normal total testosterone levels 3, 4
- In men with liver disease, SHBG rises in early stages but may decline with progression to decompensated cirrhosis 3
Differential Diagnosis
- Subclinical hyperthyroidism should be considered, with evaluation of thyroid function tests (TSH, free T4) 5
- Early liver disease should be evaluated through liver function tests, even if asymptomatic 3
- Medication effects should be reviewed, including over-the-counter drugs 1, 2
- Age-related changes, including naturally increasing SHBG levels with aging, should be considered 1, 2
