Darolutamide and Zytiga Mechanism of Action and Clinical Use

Mechanism of Action and Pharmacokinetics

  • The American Urological Association notes that darolutamide is a direct androgen receptor (AR) antagonist that blocks AR signaling, while Zytiga inhibits androgen synthesis by blocking CYP17A1, requiring concurrent prednisone administration 1
  • Darolutamide competitively inhibits androgen binding to the AR, blocks AR nuclear translocation, and prevents AR-mediated transcription, according to the European Association of Urology 2
  • Zytiga works by inhibiting CYP17A1 enzyme to block androgen synthesis, requiring concurrent prednisone 5mg daily to prevent mineralocorticoid excess, as stated by the American College of Clinical Pharmacy 1

Dosing and Administration

  • The National Comprehensive Cancer Network recommends darolutamide 600 mg (two 300 mg tablets) twice daily with food, which does not require concurrent corticosteroid therapy 3, 4
  • Zytiga is recommended at 1,000 mg (four 250 mg tablets) once daily, and must be taken with prednisone 5mg daily, according to the European Society for Medical Oncology 1, 3

Safety Profile and Adverse Events

  • The American Society of Clinical Oncology notes that darolutamide demonstrates adverse events similar to placebo in ARAMIS trial, with most common events being fatigue, pain in extremity, and rash 4
  • Zytiga has different toxicity concerns, including increased hypertension, mineralocorticoid excess if prednisone not co-administered, and higher risk of atrial fibrillation, as reported by the American Heart Association 1, 6

Clinical Efficacy Data

  • The European Association of Urology states that darolutamide in nmCRPC (ARAMIS trial) shows metastasis-free survival of 40.4 vs 18.4 months (HR 0.41; P<0.001) 4
  • Darolutamide in mHSPC (ARASENS trial) demonstrates 4-year overall survival of 62.7% vs 50.4% (HR 0.68; P<0.001), with 32.5% reduction in risk of death, according to the American Society of Clinical Oncology 1, 4
  • Zytiga in mHSPC (LATITUDE trial) shows median overall survival of 53.3 vs 36.5 months (HR 0.66; P<0.0001), as reported by the National Comprehensive Cancer Network 1

Cost Considerations

  • The Journal of Clinical Oncology notes that 6-month treatment costs for darolutamide are $72,768, while Zytiga (brand) costs $78,389.52 (1,000 mg daily) or $19,803.06 (250 mg with fatty meal), and abiraterone generic costs $9,745.56 (1,000 mg daily) or $4,953 (250 mg with fatty meal) 3

Monitoring Requirements

  • The American Urological Association recommends serial PSA every 3-6 months, conventional imaging every 6-12 months, and baseline and periodic monitoring of testosterone, LDH, hemoglobin, alkaline phosphatase for both agents 2, 4
  • Zytiga additionally requires liver function monitoring, electrolyte monitoring (potassium, sodium), and monitoring for signs of mineralocorticoid excess, as stated by the European Association of Urology 1, 6
