Postural Tremor Management

Introduction to Postural Tremor Treatment

  • The American Academy of Neurology recommends propranolol or primidone as first-line treatments for essential tremor, with propranolol having demonstrated efficacy for tremor control in up to 70% of patients 1
  • Beta-blockers, particularly propranolol, are the first-line treatment for postural tremor, with a recommended dosage range of 80-240 mg/day 1
  • Enhanced physiologic tremor can be triggered by anxiety, stress, strenuous exercise, or caffeine consumption, and lifestyle modifications such as avoiding strenuous exercise before precision tasks, reducing caffeine consumption, and implementing stress reduction techniques can help manage it 2

Pharmacological Treatment Options

  • Propranolol (80-240 mg/day) is the most established medication for postural tremor, and the American Academy of Neurology recommends it as a first-line treatment 1
  • Other beta-blockers with evidence for tremor control include nadolol (40-320 mg daily), metoprolol (25-100 mg extended release daily or twice daily), atenolol, and timolol (20-30 mg/day) 1
  • Gabapentin has limited evidence for moderate efficacy in tremor management, and may be considered as a second-line option 1

Important Considerations and Contraindications

  • Beta-blockers should be avoided in patients with chronic obstructive pulmonary disease, bradycardia, or congestive heart failure, due to potential adverse effects 1
  • Common adverse effects of beta-blockers include fatigue and depression, dizziness and hypotension, and exercise intolerance and sleep disorders, and elderly patients may experience serious adverse events from excessive heart rate reduction 1

Non-Pharmacological Approaches

  • Lifestyle modifications such as avoiding strenuous exercise before precision tasks, reducing caffeine consumption, and implementing stress reduction techniques can help manage enhanced physiologic tremor 2
  • Rhythm modification techniques, including superimposing alternative rhythms on existing tremor and using gross rather than fine movements for activities like handwriting, can help control tremor 2

Treatment for Refractory Cases

  • Surgical options such as magnetic resonance-guided focused ultrasound (MRgFUS) thalamotomy, deep brain stimulation (DBS), and radiofrequency thalamotomy may be considered for severe, medication-refractory tremor causing significant disability, with MRgFUS showing sustained tremor improvement of 56% at 4 years and lower complication rates (4.4%) 1
  • DBS is preferred for bilateral tremor, while MRgFUS is contraindicated for bilateral treatment, and the choice of surgical option should be individualized based on patient characteristics and preferences 1
