Management of Acute Cholecystitis in Pregnancy

Initial Stabilization and Diagnosis

  • The American College of Surgeons recommends beginning IV hydration and pain control immediately while preparing for definitive surgical intervention in patients with acute cholecystitis at 20 weeks gestation 1
  • Ultrasound findings, including a distended gallbladder, thickened gallbladder wall, pericholecystic fluid, gallstones, and positive Murphy's sign, are used to confirm the diagnosis of acute cholecystitis in pregnant patients 2

Why Surgery Over Conservative Management

  • The Society of American Gastrointestinal and Endoscopic Surgeons recommends surgical intervention over medical management for acute cholecystitis in pregnancy, as conservative management leads to recurrent biliary symptoms in 60% of patients and increases rates of spontaneous abortion, threatened abortion, and premature birth 1

Surgical Approach and Timing

  • The American College of Obstetricians and Gynecologists recommends that laparoscopic cholecystectomy be performed during the second trimester, as it is the optimal time for surgical intervention and has significantly lower maternal and fetal complications compared to open cholecystectomy 1

Antibiotic Coverage

  • The Infectious Diseases Society of America recommends administering antibiotics perioperatively according to severity, with Amoxicillin/Clavulanate 2g/0.2g IV q8h for uncomplicated cholecystitis and one-shot prophylaxis if early intervention within 7-10 days of symptom onset 2

Alternative Only If Surgery Contraindicated

  • The Society of Critical Care Medicine recommends percutaneous cholecystostomy tube placement as "bridging" therapy if the patient is hemodynamically unstable or at prohibitively high surgical risk, but notes that it is associated with longer hospital stays and is inferior to cholecystectomy in terms of major complications 1, 2

Expected Outcomes

  • The American College of Surgeons reports that patients undergoing cholecystectomy during index hospitalization have significantly lower cumulative hospitalizations compared to conservative management, and that same-admission cholecystectomy reduces odds of early readmission by 85% 1
