Zosyn Use in Acute Kidney Injury

Introduction to Zosyn Dosing in AKI

  • The American College of Physicians and other medical societies recommend calculating creatinine clearance using the Cockcroft-Gault formula with actual body weight, not eGFR, for medication dosing decisions in non-steady state conditions like AKI, as eGFR is unreliable and overestimates true renal function in patients with acute kidney injury 1, 2

Dosing Adjustments for Renal Impairment

  • The FDA and medical guidelines suggest that dose reduction is mandatory when creatinine clearance is impaired, with a recommended dose of 2.25 g every 6 hours for patients with CrCl 20-40 mL/min, and 2.25 g every 8 hours for patients with CrCl <20 mL/min 5

Nephrotoxicity Risk Considerations

  • Research demonstrates that the nephrotoxicity risk of Zosyn is dose-dependent and amplified in pre-existing renal impairment, with higher doses causing AKI in 25-38.5% of patients with chronic kidney disease, compared to only 5.6% with lower doses 3, 4

Essential Monitoring Protocol

  • The Kidney Disease: Improving Global Outcomes (KDIGO) guidelines recommend monitoring serum creatinine every 4-6 hours initially, then daily once stable, and strict input/output monitoring, as oliguria can indicate the need for renal replacement therapy 1, 5

Critical Actions Before Starting Zosyn

  • The National Kidney Foundation and other medical societies recommend discontinuing all nephrotoxic medications immediately, holding ACE inhibitors, ARBs, NSAIDs, and diuretics, and reviewing all medications to adjust dosages based on reduced GFR 5

When to Consider Alternatives or Delay

  • The American Society of Nephrology suggests that renal replacement therapy may be indicated if oliguria persists despite fluid optimization, severe metabolic derangements develop, uremic symptoms emerge, or fluid overload worsens, and consideration should be given to delaying empiric broad-spectrum antibiotics until dialysis access is established, if possible 5
