Diagnostic Approach to Hand Tremor in Children

Clinical Assessment and Diagnosis

  • Action tremor suggests essential tremor, enhanced physiologic tremor, or dystonic tremor as primary considerations in children, with documentation of tremor frequency, amplitude, body distribution, and response to attention or distraction helping to distinguish organic from functional tremor 1, 2
  • Medication and substance exposure, such as stimulant medications, bronchodilators, valproic acid, lithium, and caffeine consumption, can cause enhanced physiologic or drug-induced tremor, and should be inquired about specifically 3
  • Family history is highly relevant in essential tremor, which demonstrates autosomal dominant inheritance in many cases 1
  • Functional impact, including interference with daily activities or social embarrassment, guides treatment intensity 1
  • Psychological context, including recent stressors, trauma, or illness preceding tremor onset, and assessment for variable/entrainable tremor characteristics, can suggest functional etiology 2

Treatment Principles

  • Propranolol 80-240 mg/day is first-line pharmacotherapy for enhanced physiologic tremor and essential tremor, with approximately 50% of patients showing improvement 1, 3
  • Alternative beta-blockers, such as nadolol, metoprolol, and atenolol, can be substituted if propranolol is not tolerated, but should be avoided in patients with asthma, bradycardia, or heart failure 1, 3
  • Rhythm modification techniques, such as superimposing alternative voluntary rhythms and using music to override the tremor pattern, are primary treatment for functional tremor 1, 2
  • Avoid pharmacotherapy for functional tremor, as there is no evidence supporting benefit, and minimize adaptive equipment use to avoid reinforcing illness behavior 1, 2
  • Unilateral tremor with other focal neurologic deficits may indicate a structural brain lesion and requires urgent evaluation 4
