Calcium Channel Blocker Therapy with Diltiazem

Pharmacological Properties

  • Diltiazem inhibits calcium influx through L-type calcium channels in vascular smooth muscle and myocardium, leading to smooth muscle relaxation and reduced myocardial contractility, as recommended by the American College of Cardiology 8
  • It has prominent effects on atrioventricular conduction and sinus node cells, resulting in negative inotropic and chronotropic effects, according to the American Heart Association 8
  • Diltiazem is less selective for vasculature compared to dihydropyridine calcium channel blockers and has more pronounced effects on myocardial contractility, as stated by the European Society of Cardiology 9

Clinical Uses and Considerations

  • Diltiazem is particularly effective in vasospastic angina when used alone or in combination with nitrates, as recommended by the American College of Cardiology 8
  • Diltiazem should be avoided in patients with pulmonary edema or severe left ventricular dysfunction, according to the American College of Cardiology 10, 11
  • Concomitant use with ivabradine is contraindicated due to risk of severe bradycardia, as warned by the European Medicines Agency 8
  • Caution is needed when combining diltiazem with beta-blockers due to risk of high-degree atrioventricular block, as advised by the American Heart Association 12

Side Effects and Comparative Efficacy

  • Major side effects include hypotension, worsening heart failure, bradycardia, and AV block, as reported by the Journal of the American College of Cardiology 10
  • Other common side effects include headache, dizziness, flushing, and leg edema, according to the European Heart Journal 8
  • Diltiazem reduced reinfarction and refractory angina without increasing mortality rates in studies of unstable angina/NSTEMI, as found by the American College of Cardiology 10

Clinical Decision Making

  • For patients with high heart rate (>70 bpm), heart-rate-lowering agents like diltiazem may be preferred, as recommended by the European Society of Cardiology 12
  • For patients with vasospastic angina, calcium channel blockers including diltiazem are particularly effective, as stated by the American College of Cardiology 8, 9
  • In patients with chronic obstructive pulmonary disease with positive bronchoreactivity or asthma, diltiazem may be a preferred option, as suggested by the American Thoracic Society 13
