Differentiating NSTEMI from Acute Heart Failure Using Troponin Levels
Key Diagnostic Features
- The American College of Cardiology and European Heart Society recommend serial troponin measurements to differentiate NSTEMI from acute heart failure, with a characteristic rise and fall pattern and at least one value above the 99th percentile, along with clinical evidence of myocardial ischemia 1, 2
- Serial cardiac troponin measurements should be obtained at presentation and 3-6 hours after symptom onset in all patients with suspected ACS, with high-sensitivity troponin assays allowing for measurements at 0h and 1h (or 2h) 1, 3
- Elevations beyond 5-fold the upper reference limit have high (>90%) positive predictive value for acute type 1 MI, according to the European Heart Journal 3
Diagnostic Algorithm
- The European Heart Journal recommends evaluating clinical presentation, 12-lead ECG, and initial cardiac troponin, and considering pre-test probability of coronary artery disease versus heart failure exacerbation 5, 6
- Serial troponin measurements should be obtained at presentation and 3-6 hours after symptom onset, with high-sensitivity assays allowing for measurements at 0h and 1h (or 2h) 1, 3
- A rising and/or falling pattern with at least one value above the 99th percentile is suggestive of NSTEMI, according to the European Heart Journal and Journal of the American College of Cardiology 2, 4
Important Caveats
- Troponin elevation occurs in both NSTEMI and acute heart failure, and is not specific to either diagnosis, according to the European Heart Journal 7, 8
- Multiple conditions other than NSTEMI can cause troponin elevation, including tachyarrhythmias, hypertensive emergencies, critical illness, renal dysfunction, pulmonary embolism, myocarditis, Takotsubo syndrome, and other non-ischemic cardiac conditions 3, 7, 8
Clinical Implications
- Additional troponin measurements beyond 6 hours should be obtained in patients with normal initial values when clinical suspicion for ACS remains high, according to Circulation 1
