BTK Inhibitors and Pneumonia-Related Effusions
Key Safety Concerns with BTK Inhibitors and Pneumonia
- Acalabrutinib demonstrates a lower pneumonia rate of 7% (grade 3-4 in 5% of patients) compared to ibrutinib, as recommended by the National Comprehensive Cancer Network 1, 2
Clinical Management Approach
- The first-line diagnostic approach for suspected pneumonia should include a chest CT scan, inflammatory markers, urinary antigens for Legionella pneumophila and Pneumococcus, two sets of blood cultures, nasopharyngeal swab for respiratory viruses including SARS-CoV-2, sputum culture if present, and serum galactomannan and beta-D-glucan if aspergillosis or pneumocystosis suspected, as suggested by Blood Reviews 3, 4
- The second-line diagnostic approach if no etiology identified or lack of response should include bronchoalveolar lavage with comprehensive microbiological assays, molecular analysis for atypical bacteria, CMV-DNA, and fungi, BAL fluid galactomannan and fungal PCR, and brain MRI and lumbar puncture if central nervous system dissemination suspected, as recommended by Blood Reviews 3, 4
Prophylaxis Recommendations
- Routine antibiotic prophylaxis is NOT recommended for patients on BTK inhibitors, but early clinical suspicion, rapid diagnosis, and appropriate therapy are preferred over prophylaxis, as suggested by Blood Reviews and the National Comprehensive Cancer Network 3, 4, 5
- Routine mold-active antifungal prophylaxis is NOT recommended for patients on ibrutinib or other BTK inhibitors in the absence of additional risk factors, but consider prophylaxis only in high-risk scenarios, as recommended by Blood Reviews 3, 4, 5
Important Clinical Pitfalls
- Ciprofloxacin significantly increases plasma concentrations of ibrutinib and venetoclax, requiring dose adjustments, as reported by Blood Reviews 3, 4
- Azole antifungals have significant drug-drug interactions with BTK inhibitors through CYP3A4 inhibition, leading to increased ibrutinib levels and potential toxicities, as warned by Blood Reviews 3, 4, 5
