Topical Antibiotic Use for Minor Cuts and Scrapes

Indications and Contraindications

  • The Infectious Diseases Society of America recommends that if signs of infection develop, such as redness, warmth, purulence, or increasing pain, topical antibiotics should be discontinued and systemic therapeutic antibiotics initiated 1, 2
  • The American College of Clinical Pharmacology suggests that topical antibiotics are not appropriate for deep or puncture wounds, animal bites, serious burns, or contaminated wounds from field injuries, and instead require therapeutic-dose systemic antibiotics 1, 2
  • The Clinical Infectious Diseases guidelines state that wounds with established infection require systemic therapeutic antibiotics for 1-2 weeks for mild infections, or 2-4 weeks for moderate-to-severe infections 1, 2

Wound Management

  • Proper wound care, including thorough cleansing and appropriate wound management, is essential and should not be substituted with topical antibiotics alone 1, 2
  • The Clinical Infectious Diseases guidelines recommend recognizing that topical antibiotics are only indicated for superficial, clean, or mildly contaminated wounds, and more severe wounds require systemic therapy 1, 2
