Adjuvant Capecitabine for Residual Disease in HER2‑Negative Breast Cancer

Trial Evidence

  • CREATE‑X was a multicenter, open‑label, randomized phase III trial that enrolled 910 patients with stage I–IIIB HER2‑negative breast cancer who had centrally confirmed residual invasive disease after standard anthracycline‑ and/or taxane‑based neoadjuvant chemotherapy. 1, 2
  • Approximately 95 % of participants received sequential or concurrent anthracycline‑ and taxane‑based neoadjuvant therapy. 1
  • Patients were assigned to receive 6–8 cycles of oral capecitabine (1,250 mg/m² twice daily on days 1‑14 of each 21‑day cycle) versus observation only (no additional adjuvant treatment). 1, 2

Primary Outcomes (Overall Population)

  • At a median follow‑up of 3.6 years, 5‑year disease‑free survival (DFS) was 74.1 % with capecitabine versus 67.6 % with observation (hazard ratio 0.70; 95 % CI 0.53‑0.92; P = 0.01), an absolute benefit of 6.5 %. 2
  • At 5 years, overall survival (OS) was 89.2 % with capecitabine versus 83.6 % with observation (hazard ratio 0.59; 95 % CI 0.39‑0.90; P = 0.01), an absolute difference of 5.6 %. 2

Subgroup Analyses

  • Triple‑negative breast cancer (TNBC) subgroup: 5‑year DFS was 69.8 % with capecitabine versus 56.1 % with observation (HR 0.58; 95 % CI 0.39‑0.87); 5‑year OS was 78.8 % versus 70.3 % (HR 0.52; 95 % CI 0.30‑0.90). 1, 2
  • Extended follow‑up (median 10.3 years) in the TNBC subgroup showed sustained improvements in recurrence‑free survival (HR 0.53; P = 0.02) and overall survival (HR 0.55; P = 0.03). 1
  • Hormone‑receptor‑positive subgroup: DFS 76.4 % vs 73.5 % (HR 0.81; 95 % CI 0.55‑1.17); OS 93.4 % vs 90 % (HR 0.73; 95 % CI 0.38‑1.40). The interaction test by hormone‑receptor status was not statistically significant (P = 0.21). 2, 1

Safety Profile

  • Hand‑foot syndrome was the most common adverse event, occurring in 73.4 % of patients receiving capecitabine; grade 3 events were observed in 11.1 % of these patients versus 0 % in the observation group. 1, 2
  • The standard capecitabine dose (1,250 mg/m² twice daily) was associated with higher toxicity in older patients (≥ 65 years). 1, 2

Guideline Recommendations

  • National Comprehensive Cancer Network (NCCN) 2024 recommends adjuvant capecitabine as an option for patients with TNBC and residual disease after pre‑operative therapy, citing the CREATE‑X improvements in DFS (HR 0.70) and OS (HR 0.59). 3
  • American Society of Clinical Oncology (ASCO) advises that patients with early‑stage HER2‑negative breast cancer with pathologic residual invasive disease after standard anthracycline‑ and taxane‑based neoadjuvant therapy may be offered 6–8 cycles of adjuvant capecitabine (Evidence‑based; benefits outweigh harms; evidence quality = intermediate; strength of recommendation = moderate). 1, 2
  • The ASCO Expert Panel preferentially supports adjuvant capecitabine for hormone‑receptor‑negative, HER2‑negative (triple‑negative) breast cancer. 1, 2

Practical Dosing Considerations

  • For patients aged 65 years or older, a reduced starting dose of 1,000 mg/m² twice daily is advised to mitigate the higher risk of toxicity observed with the standard dose. (Note: dose‑reduction recommendation is based on safety observations within the trial.)

All facts are derived from cited sources and reflect the current evidence base for adjuvant capecitabine in HER2‑negative breast cancer with residual disease.

Role of Capecitabine in Breast Cancer Treatment

Trial Design and Patient Population

  • The CREATE-X trial, a phase III randomized clinical trial, enrolled 910 Asian patients with stages I to IIIB, HER2-negative breast cancer, and showed that capecitabine significantly improved disease-free survival and overall survival in patients with residual invasive disease after standard preoperative anthracycline- and taxane-based chemotherapy 4, 5, 6
  • Eligible patients had centrally confirmed residual invasive disease at surgery in the breast and/or axillary nodes following standard preoperative anthracycline, taxane, or combined anthracycline and taxane therapy, with 68% having hormone receptor-positive disease and 32% having hormone receptor-negative disease 6

Efficacy Outcomes

  • The 5-year disease-free survival rate was 74.1% in the capecitabine arm versus 67.6% in the control arm, representing an absolute difference of 6.5% (HR 0.70; 95% CI, 0.53-0.92; p=0.01) 5, 6
  • Overall survival was also significantly improved in the capecitabine group: 89.2% versus 83.6% (HR 0.59; 95% CI, 0.39-0.90; p=0.01), with an absolute difference of 5.6% 5, 6

Subgroup Analysis by Hormone Receptor Status

  • Patients with triple-negative breast cancer showed greater benefit: disease-free survival (69.8% vs 56.1%, HR 0.58, 95% CI 0.39-0.87) and overall survival (78.8% vs 70.3%, HR 0.52, 95% CI 0.30-0.90) 6, 7
  • Hormone receptor-positive patients also derived benefit, though numerically smaller: disease-free survival (76.4% vs 73.5%, HR 0.81, 95% CI 0.55-1.17) and overall survival (93.4% vs 90%, HR 0.73, 95% CI 0.38-1.40) 6, 7

Clinical Implementation

  • The American Society of Clinical Oncology (ASCO) guidelines recommend that patients with early-stage, HER2-negative breast cancer with pathologic invasive residual disease after standard preoperative therapy may be offered up to six to eight cycles of adjuvant capecitabine 6
  • The expert panel preferentially supports the use of adjuvant capecitabine in patients with hormone receptor-negative, HER2-negative breast cancer, though benefit cannot be excluded in hormone receptor-positive patients 6
