Evaluation and Management of Tongue Fasciculations in Amyotrophic Lateral Sclerosis

Diagnostic Evaluation

  • The American Academy of Neurology recommends assessing for associated bulbar symptoms, including dysarthria, dysphagia, drooling of saliva, and muscle weakness in other body regions, as these symptoms are commonly found in ALS patients 1
  • The European Society for Clinical Nutrition and Metabolism suggests screening for dysphagia at every visit, as oropharyngeal dysphagia affects 47.8-72.7% of ALS patients and leads to aspiration pneumonia, malnutrition, and reduced quality of life 1

Management Approach

Nutritional Monitoring and Support

  • The American College of Nutrition recommends screening for malnutrition at diagnosis and every 3 months using BMI and weight loss measurements, and targeting weight gain in patients with BMI <25 kg/m² 1
  • The Clinical Nutrition society suggests monitoring for refeeding syndrome risk by checking baseline phosphate, magnesium, and potassium before aggressive nutritional support, particularly in elderly patients with poor intake, although this is based on a guideline from Praxis Medical Insights, a more relevant citation is not available 4

Dysphagia Management

  • The American Thoracic Society recommends performing videofluoroscopic or endoscopic swallowing evaluation before and after therapeutic interventions to objectively assess swallowing biomechanics 5
  • The European Society for Clinical Nutrition and Metabolism suggests considering early gastrostomy tube placement when dysphagia progresses to requiring dietary consistency changes or tube feeding, as this is preferable to prolonged nasogastric feeding 1

Critical Differential Diagnoses

  • The International Parkinson and Movement Disorder Society recommends excluding functional neurological disorder by looking for internal inconsistency of symptoms, resolution with distraction, and disproportionate severity relative to examination findings, as this can mimic ALS symptoms 2

Common Pitfalls to Avoid

  • The American Academy of Neurology recommends not delaying dysphagia screening until symptoms become severe, as early intervention prevents aspiration pneumonia and maintains nutritional status 1
